Graduate Waiver Maintenance Form
This form must be completed between January 1 and March 31 each academic year by HSTA Scholars who wish toeither defer or continue receiving a HSTA waiver. It must also be completed any time a HSTA Scholar transfers between schools, re-enrolls for school, or changes her or his educational status. If you aren't using a waiver, you can use this form to update us with your current school or work status and contact information as well.
When you submit your form, you agree to the following: "I give HSTA permission to use my GPA and test scores for program evaluation purposes. I understand this information will remain strictly confidential."
