WV Improving the Health of Americans through Prevention and Management of Diabetes, Heart Disease and Stroke -1815

OHSR continues efforts to support chronic disease quality of care improvement in primary care centers statewide. The OHSR assists primary care centers and community-based programs in accurately tracking patient outcomes, benchmarking care against national standards, modifying clinical policies and procedures for improved outcomes, integrating team-based care and developing and sustaining clinic/community linkages for chronic disease prevention and control through the West Virginia Health Connection initiative. West Virginia Health Connection is an initiative led by OHSR designed to support clinical/community linkages for chronic disease prevention and management.
