What Can You Expect from Care Navigation?

  • A designated Care Navigator is a peer who is also in recovery that has received training to help patients with life challenges in recovery.

  • A focus on holistic health, services that meet the need of the patient–mentally, emotionally, physically and spiritually.

  • Help in finding hope and getting the most out of the care experience, including utilizing community resources related to housing, transportation, food, recovery-based meetings, and working through any other social determinants that may exist.

How Do Care Navigators Support You?

  • Teach about the recovery process and helping with pre-recovery planning.

  • Provide support by continued phone, zoom meetings, or in person interactions.

  • Provide wellness information and access to the community resources patients are seeking for individualized needs.

  • Help the patients meet others who are working toward similar wellness goals.

  • Empower the patients come up with their own solutions.

  • Support the patients follow through with their decisions.
