Transitions of Care Clinical Experience

Prerequisite: IPE IDEAS - Through the implementation of our “Interprofessional Huddles" in the Transitions of Care Clinic, learners from various healthcare disciplines are not only exposed to live telemedicine clinical encounters but also contributed to multidisciplinary interventions that improved health outcomes for high-risk rural patients following hospital discharge.
Although many learners have previous telemedicine experience, most following participation felt that they had improvements in their general understanding of telemedicine, more likely to find telemedicine as most effective when performed in a multidisciplinary team setting and felt that they were more likely to incorporate telemedicine in their future practice.
The patients enrolled in this grant-funded clinic are selected due to their many healthcare barriers and low access to healthcare resources and were treated by multiple healthcare disciplines that they rarely benefit from after hospital discharge. The interventions crafted by learners and clinic providers had lasting impacts on the healthcare outcomes of the patient’s seen in the clinic and revealed the power of using interprofessional education as a tool not only for students but for improved patient care. Certificates of completion will be offered.
