Transitions of Care Clinical Experience

Many learners entered the Transitions of Care Clinical Experience with some telemedicine experience, but most left with a deeper understanding of how it works. Participants said they now view telemedicine as most effective in a team-based setting and are more likely to use it in their future careers.

Patients in this grant-funded clinic are selected because they face significant healthcare barriers and have limited access to resources. Through this program, they receive care from multiple healthcare disciplines—support they often don’t have after hospital discharge.

The interventions developed by learners and clinic providers made lasting improvements in patient outcomes and demonstrated the value of interprofessional education not only for students but also for enhancing patient care. Certificates of completion are available.

Format: In-Person/Clinic
Time: 1 hour
Intended Audience: Recommended for clinical level learners and can only accommodate between 1-2 learners per discipline per session.
Prerequisite: IPE IDEAS - Through the implementation of our “Interprofessional Huddles" in the Transitions of Care Clinic, learners from various healthcare disciplines are not only exposed to live telemedicine clinical encounters but also contributed to multidisciplinary interventions that improved health outcomes for high-risk rural patients following hospital discharge.
