ACT/SAT
All HSTA students are eligible for reimbursement of basic registration fees (plus taxes) for two ACT or SAT tests. Only ACT and SAT tests qualify for reimbursement. HSTA will NOT cover the cost of add-on tests, late fees, rush fees, standby testing, change fees, cancellation fees, additional college or score reports, archived scores, scores by phone, ACT My Answer Key, or score verification.
To receive reimbursement, please complete the form linked below. After the form is submitted, the person whose contact information is provided will receive an email from WVU Supplier Registration with instructions for completing and uploading a W-9. Anyone who paid the registration fee (the student, parent, etc.) can be reimbursed, but both this form and the W-9 must be completed with the name and contact information of that person. A copy of the receipt (showing the student’s name, test date, and a breakdown of charges) and a copy of the scores (showing the student’s name, test date, and scores) must be attached when submitting this form.
